This includes those who are eligible for fully funded NHS Continuing Health Care (CHC), or are in receipt of a joint funded package of care and have complex needs. The service must be able to receive and response to referrals 7 days a week during the hours of 7:30am and 10:30pm. The service is to be provided within a person's own home. The majority of activity supports CHC fast track patients discharged from hospital in the last phase of life.
