This ambition includes establishing a primary focus on customer outcomes being met by the provision of time-limited 'Step-up' and 'Step-down' services aimed at recovering independence and resident ability to continue to stay in their community for longer. The ASC ambition also includes establishing early, the provision of step-up support to enable customers to remain at home or in the community, to be at least equitable with step-down provision, contributing to the realisation of ASC priorities in prevention and early intervention and reducing the cost of ongoing care packages whilst improving outcomes for residents. This work should result in setting out the early changes required for an optimised reablement function that aims to avoid or reduce the need for long-term ongoing support for residents, where possible and safe, with defined referral pathways from Social Work Neighbourhood Teams, Careline and Hospital Discharge processes.",
