By exploring new models of care - within general practice and elsewhere - we can work with health communities across England to consider which of the new options signalled by the Five Year Forward View constitute viable ways forward for local health and care services over the next five years and beyond. And as we introduce them, we also need to evaluate new care models to establish which produce the best experiences for patients and the best value for money.",
