These chosen areas are based on the highest levels of South Asian populations matched to an increased prevalence of diabetes This is a contract in 3 lots with a total cost of 40,000 per annum. Lot 1 - Harehills/Chapeltown - 14,000 Lot 2 Beeston& amp; nbsp; - 13,000 Lot 3 - Gipton/Oakwood - 13,000 The total length of the contract including design and the post contract evaluation will be 12 months. Organisations are encouraged to apply for one, all or any combination of the lots available. Please refer to Part 2 Specification document for full service requirements.",
