With a focus on frontline behaviours, practice and ways of working, the improvement work set out in this proposal seeks to ensure patients admitted to inpatient mental health beds are discharged as early as possible once they are deemed Clinically Ready for Discharge (CRFD) on the appropriate discharge pathway that maximises their independence and likelihood of requiring follow-up emergency or acute intervention.",
