This should consider the clinical models and input from different professions. * Whether the pilots have improved health and social care outcomes (measurable and actionable) in the short term? * Whether they have reduced costs and improved system efficiencies? * Whether/how the model supports the recommendations within the new NICE clinical guidance (NG27) \"Transition between inpatient hospital settings and community or care home settings for adults with social care needs\".
