This contract is initially for 3 years with the option to extend for an additional 2 years. The provider will work with the service users to: Maintain tenancies. Provide person-centred support, which takes full account of diverse needs and lifestyle choice. Promote Client's choice, well-being and safety. Promote people's independence by focusing on abilities and encouraging people to be as self-directing as possible. Manage the home environment. Manage finances, maximise income/benefits. Encourage access to employment/education. Encourage meaningful day-time activities, community access and integration. Engage with relevant professionals, i.e., Mental Health Team, Housing Options Officers, Employment agencies Establish and sustain effective social support.
