Clinical coding is the translation of clinical information contained in the case notes regarding a patient's diagnosis and treatment into a standardised coded format which is nationally and internationally recognised, using the following classifications: ICD-10 5th Edition- International Classification of Diseases devised by the World Health Organisation and contains codes that cover all reasons for the patient being admitted to hospital. OPCS 4.7 - Office of Population Censuses and Surveys, 4th Revision devised by the National Classifications Service which is a national publication and covers all operative procedures and interventions. To ensure completeness of coding the Clinical Coder has access to many systems such as pathology, radiology, maternity, renal and maternity clinical systems.
