This service will enable non-housebound patients with wounds that require more specialist care, to achieve improved quality of life and independence. To review and where appropriate treat patients referred by Tier 1 service or discharged by Tier 3 Wound Care services by providing: * Specialist advice and service provision; * High quality and safe-care using evidence-based interventions; * Good access to services and education; * Responsive patient-centred care.
