        "description": "Haringey Clinical Commissioning Group (HCCG) and London Borough of Haringey (LBH) are currently reviewing support services for individuals who have experienced and survived stroke with a view to re commissioning a new service by April 2017. Our joint aim is to commission a support service that promotes independence and enhances the health and wellbeing of individuals who have been affected by a stroke and their carers so that they can participate as full citizens in the community. The service will be focused on supporting individuals who have been affected by a stroke and their carers to access everyday activities such as leisure, education, training and work (paid and unpaid). This is especially important in preventing the more serious psychological consequences of a stroke and in fostering self-reliance and healthy living after stroke. The service will support people and their carers to navigate the services and support available across the system and work in close partnership with a wide a range of statutory and non-statutory agencies in order to ensure improved outcomes and user experience. Key Elements of the Service * Six-month stroke reviews * Stroke club functions * Stroke navigator function * Facilitated and un-facilitated peer support groups We would like to invite you to a market event to help us shape our thinking about the new stroke support service.
