The review must cover the following areas: * Medicines, secondary prevention strategies, general health needs * Mood, memory, cognitive and psychological status * On-going therapy and rehabilitation needs * Social care needs, benefits and finance, driving, and transport * Return to work/vocational needs * Needs of the carer(s)",
