        "description": "NHS Wakefield Clinical Commissioning Group (CCG) is seeking to procure the provision of bereavement support for the population of the Wakefield District offering a 3 year contract. The Service is for those who have been closely affected by a death and require additional support in learning to live with their grief following bereavement. The purpose of the Service is to ensure that timely and relevant one to one and group bereavement support is available to adults registered with a Wakefield General Practice (GP) who have been closely affected by a human death. Furthermore the Service will be in a position to identify those at increased risk of, or currently experiencing a complex or prolonged grief reaction and support them to access appropriate services. The Service will be provided by a number of trained bereavement support staff/volunteers who offer the following range of services: * Triage/Assessment * One to One bereavement support * Bereavement Support Groups * One to One Intensive Bereavement Support * Provision of information/Signposting The Service Providers will need to work closely with other local agencies and services that have a role within the management of death within the community, particularly primary care teams. This will be an open tender for a single service provider. Additional information: It is anticipated that this tender will open on the 7 January 2020.
