This new model of care will redesign and reorganise the existing community mental health resource to form new community mental health hubs each of which will be aligned to the local GP Primary Care Networks (populations of 30,000- 50,000). The community mental health hubs will have blended teams of health, social care and VCFSE staff; working together, and with local communities, to improve mental and physical health.
