Integration, collaboration, and service sustainability - The provider integrates and collaborates with partners. The provider works collaboratively with other services / agencies. The current and future service model is considered sustainable. Improving access, reducing health inequalities, and facilitating choice - This is considered of high importance due to the patient population. This is expected to be maintained in the new contract.
