Summary List of the Scope Requirements is as follows: The Service will provide accessible information and support services for those aged 18 years and over, focusing on supporting carers resilience, wellbeing and enabling them to thrive. In brief, the Adult Unpaid Carers Support Service will include: Assessment (Care Act and Holistic): ensuring Care Act compliance and support planning. This includes Carers Breaks. Carers identification /discount Card: to support carer identification and wellbeing. Carers Partnership Board and Carers Response Groups: administration, coordination and facilitation Collaboration and partnership working to form strong partnership working across the Integrated Care System and communities to promote Carer Aware. Contingency planning: to support carers with their own plans as a priority including Carers Emergency Scheme. Health and Wellbeing: peer support, counselling, community engagement Information, Advice and Guidance Training, Skills, Activities & Development: increasing Carer resilience and wellbeing.
