AIMS AND OBJECTIVES * To prepare individuals with the necessary skills to transition successfully into independent living. * To support individuals to live independently and move onto alternative, affordable long term housing. This alterative housing may include shared properties, private rented accommodation, social housing etc. * To assist clients with their presenting support needs, offering a harm reduction approach and ensuring referrals to relevant agencies are made to meet medical and physical needs (e.g., Specialist Health Team). * To provide all residents with support to help identify their needs, expectations, and aspirations. * To provide a range of planned activities that are designed to help clients develop their personal skills to live independently and to secure and manage a home. * To provide support that is person centred, taking into account their personal needs, interests, and individual goals. * To work in partnership with Housing, Substance Misuse providers, Health provision, Adult Social Care, training, employment agencies and Criminal Justice Services to deliver holistic support packages. * To ensure clients are referred and supported by the agencies that can most effectively and appropriately meet their needs.
