Community Engagement and Social Activities - Social clubs, peer-led support groups, and volunteering opportunities to reduce isolation and build social networks. 3. Support for Independence - Employment workshops, life skills training, and practical support for carers and families. 4. Empowerment - Income maximisation, digital inclusion, and proactive signposting to relevant services. Delivery Model: * The service will be community-based, operating from multiple venues across Havering to ensure local accessibility. * The provider will act as a central coordination point, managing referrals, partnerships, and performance monitoring. * Collaboration with specialist organisations is encouraged to ensure comprehensive and expert-led service delivery.
