The preferred Provider (Most Suitable Provider) will need to: * Hold a current contract for delivery of Primary Care Medical Services (Alternative Provider Medical Service (APMS), General Medical Service (GMS), or Personal Medical Service (PMS)) in North East Lincolnshire or North Lincolnshire; * Hold a valid current CQC registration for Primary Care Medical Services; * Ensure healthcare professionals delivering the services are registered with the associated registration bodies - General Medical Council, and Nursing and Midwifery Council The service will be required to: * Delivery primary care medical services, as defined by the APMS Directions - Alternative Provider Medical Services Directions, and Specification to patients registered on the Special Allocation Scheme programme. * Support patient rehabilitation and facilitate a return to General Practice registration.",
