Over the course of engagement we aim to establish a shared understanding of what is possible and desirable across a broad spectrum of services, including but not limited to: * Community-based health and care services * Rehabilitation and reablement services * Preventative and proactive care models * Integrated and multidisciplinary approaches * Digital and Place based innovations For clarity, future service provision will be delivered for the benefit of citizens registered with GP practices in the whole of the county of Surrey (the current NHS Surrey Heartlands ICB footprint and the incoming areas of Surrey Heath and Farnham currently part of the NHS Frimley ICB footprint but subject to the ICB boundary change from April 2026) either from service commencement, or in a managed and phased manner.
