CVD is the leading cause of death in the UK. Almost three quarters of cases are due to modifiable risk factors and rates of premature mortality from heart disease and stroke are higher among those living in the most deprived communities. There is a major opportunity in enabling patients to exercise greater control over their health through supportive environments, in early detection of conditions such as high blood pressure, type 2 diabetes, chronic kidney disease and raised cholesterol, and in proactive management of risk, particularly in more deprived groups. By strengthening prevention, enhancing timely diagnosis and management, and ensuring high quality treatment, rehabilitation, and long-term care for those who develop CVD, the system can meaningfully improve outcomes and help more people live longer, healthier lives. The focus of any future collaboration will include, but is not limited to, the government's ambition to reduce premature deaths from heart disease and stroke by 25% within a decade. This is not a public procurement or sponsorship opportunity and will not result in the award of a \"public contract\" (within the meaning of the Procurement Act 2023). DHSC is specifically looking to collaborate on a \"contribution in kind\" basis and wishes to hear from interested parties who can help us with: * Supporting people to lead healthier lives, for example through healthy food environments and physical activity * Raising awareness of CVD and its risk factors, including links to cardiometabolic and renal conditions. * Improving case-finding of CVD risk factors like high-blood pressure and raised cholesterol, for example by offering checks in suitable locations * Community-based rehabilitation opportunities Engagement process and deadline Those wishing to participate in the engagement process should register their interest by completing and submitting the questionnaire at the following link: https://forms.office.com/e/ywSUqzvRpk. Responses received after this time will not be accepted. We will be using a fair and non-discriminatory evaluation method to select interested parties to enter into discussions with regarding a potential collaboration agreement, following the close of the market engagement process. DHSC intends to select up to two (2) organisations to collaborate with, but it reserves its right to select fewer or more organisations and/ or not form a collaboration agreement on the basis of responses to this market engagement. Scope of requirements We recognise that collaboration across sectors can help address the challenges surrounding CVD. We are open to a range of partnership models and are particularly interested in contributions that may include: * Financial or in-kind support * Donations of goods or services * Joint public awareness campaigns * Access to networks, data, or expertise * Programme suggestions/development * Other innovative resources or approaches In return, DHSC may: * Collaborate with you around the publication and implementation of the CVD Modern Service Framework. * Collaborate with you around PR moments and provide recognition to the collaboration. * Seek opportunities to promote outputs from the collaboration.
