Referrals to the service may be from health, social care professionals or other agencies who identify a benefit or while they recover from illness, injury, or after a stay in hospital. This provision will offer individuals and their carers, choice, support improvements in quality of life, and act to deliver specific and appropriate interventions. It will provide support (and to any carers as appropriate) through the time of difficulty, enabling people to remain independent and preventing the worsening of the presenting issue. In this way, unnecessary referrals to formal emergency and crisis support services or commissioned health and care services will also be avoided.",
