This is achieved by ensuring consistent practice within schools through access to specialist teachers and/ or practitioners and further through our pupil tracking requirements for children with SEND who may also have a social worker or be cared for by the Council. Outreach is a collective term which covers: * undertaking an assessment of needs * providing information, advice and guidance to teachers, Learning Support Assistants, (LSAs) and parents * identifying specific interventions which can be modelled through teaching or coaching * recommending support programmes and signposting to other services * planning for progression and joint working, * sharing best practice and delivering whole school or individual training.
