"procurementMethodRationale": "The hospital's current surgical endoscopy equipment has developed over several decades into a collection of incompatible, outdated, and irreparable systems. These systems can no longer be serviced and are now classified as obsolete. The objective is to consolidate the equipment into a single, multi-speciality visualisation platform with essential features, such as a fluorescence (Near Infrared - NIR) unit. This platform must meet the requirements for joint surgery while also addressing thoracoscopic and laparoscopic needs, with the added benefit of reducing service and maintenance costs. The equipment was required to meet the following critical criteria: Sentinel Lymph Node Mapping and Intraoperative Localization: When used with Indocyanine Green (ICG) injection, allows for rapid and accurate identification of sentinel lymph nodes, significantly reducing surgical time. This application is crucial in procedures requiring the detection of metastatic nodes, enhancing the accuracy of cancer surgeries. Gastrointestinal Tract Viability: By injecting ICG intravenously, this provides real-time visualisation of vascularisation in the GI tract within 20-30 seconds. Non-viable areas are easily identified as they do not fluoresce, enabling surgeons to make informed decisions during procedures such as Gastric Dilatation-Volvulus (GDV) and volvulus surgeries. Tumour Margin Identification: Administering ICG 24 hours before surgery, particularly for liver tumours and insulinomas, highlights tumour margins effectively. This method shows promise for future applications, including conjugation with tumour-specific markers for enhanced fluorescence and precision in tumour resection. Skin Flap Viability: Confirming the viability of skin flaps after raising and placement. Injecting ICG intravenously helps verify that flap edges are viable and not kinked, thus ensuring successful outcomes in reconstructive surgeries.",

These developments represent a valuable addition to surgical capabilities, particularly in cancer surgery, which is a significant portion of the Hospital's soft tissue surgical caseload.",
