The contract, expected to start on 1 April 2020, has an anticipated total value of £400,000 over a maximum of five years. The procurement method follows an open procedure, with expressions of interest due by 12pm on Monday 28 October 2019. The budget is structured across multiple financial years, allocating £80,000 annually from 2020-2025. The WCCG's emphasis on collaboration with various sectors, including voluntary organisations, means that small and medium-sized enterprises as well as voluntary and community sector entities may find a strong fit in delivering the required services. ## Notice NHS Wandsworth Clinical Commissioning Group (WCCG) wishes to invite expressions of interest and a willingness to engage as the WCCG seeks to commission a new local service to support patients with Dementia. The WCCG has developed a new service model which will play a central role in providing systematic and proactive support to people and their carers/family who are living in the community with dementia through personalised case management. This new service will work in close collaboration with health and social care and the voluntary sector and be a single point of access linking all dementia support in the borough. The Dementia Support Service will support people diagnosed with dementia, and also their carers, families and those close to them to seek help, advice and support at any stage of the illness and will provide a single identifiable point of contact. Based on the Memory Assessment Service performance data, from 2016 to 2018, an average of 419 people per year have been diagnosed with dementia (based on organic primary diagnosis only)'. The intended key service outcomes are as follows: * People with dementia and their carers/families can easily access peer support, education and a range of social and leisure activities that improve well-being; * People diagnosed with dementia and their carers are provided with appropriate advice and support at the appropriate time; * People with dementia can remain in their own homes and are independent for longer; * Informal carers of people with dementia receive services that support their own health and well-being which will enable them to continue the caring process for longer; * Interactive approaches are taken so that residents with dementia living alone and those facing travel difficulties are supported to get access to services on time; * The service works in partnership with other organisations/services (e.g. Memory Assessment Service, Carers Support Service, social services, voluntary sector organisations, GP Practices, Primary Care Networks, Make Every Contact Count and Wandsworth Social Prescribing Service etc.) to meet the needs and set outcomes for service users and their carers in a complementary manner; * Increased numbers of dementia champions and dementia friends. The current assumptions are that the annual contract value will be circa PS80,000, and that the WCCG is likely to establish a contract for an initial 3 years (worth PS240,000), with an option to extend this for a further 2 year, i.e. a maximum of 5 years (worth up to PS400,000 in total). The WCCG would like for the new service to commence from 1 April 2020.
