We are looking for providers to undertake robust and legally compliant strength-based reviews/re-assessment for all Adults 18+, including older people, and those residents that may have a learning disability and/or complex health needs including mental health, funded by Hackney Adult Social Care Department and/or East London Foundation Trust or Hackney and City CCG.
