## Notice

The provider will offer a community-based skin surgery service will integrate fully with primary care, the patients' registered GP and the local hospital based MDTs where appropriate. The provider can elect to provide all or any one of the following specifications of service, namely: - Specialist skin surgery - Ophthalmic surgery - Non-scalpel vasectomy The commissioners requires a responsive community-based service model with the overall aim to provide routine, clinically appropriate diagnosis and treatment of acute conditions requiring a certain level of skin surgery and support for primary care clinicians in the management of patients. The service will have the following aims, providing services for patients with conditions requiring skin surgery that: a) Follow evidence-based guidelines and independent quality standards for skin surgery b) Are timely and patient-centred providing the most appropriate method of treatment for each patient c) Aid early diagnosis and management of patients with a condition requiring skin surgery and avoid unnecessary referral to secondary care, while providing high quality services in the community setting. It should be noted that under the Any Qualified provider model, these will be zero value contracts with no guarantee of the volume of activity. All activity will be paid monthly on a cost per case basis, retrospectively.
