The event will be held on 26th October 2018 between 10.30am and 12.30 . Full details of the venue are included via the Eventbrite booking system, details of which are included below. The service will provide: a) assessment and review of adults who may be eligible for CHC and the placement and on going contract management of adults who are hose in receipt of a care package , including PHBs b) assessment and review of Children and Young People who may be eligible for continuing care and, with partner agencies, the placement and on going contract management of those in receipt of a care package, including PHBs c) assessment and review of individuals who may be entitled to ongoing NHS funding where core service provision cannot meet their on going needs, and with partner agencies, the placement and on going contract management of those in receipt of a care package, including PHBs d) assessment and review of individuals who require a period of slow stream rehabilitation following an acquired brain injury where the individual has not reached their potential, and with partner agencies, the placement and on going contract management of those in receipt of a care package e) an element of case management of all the above A draft service specification is attached to this advert for potential providers to consider.
