These beds will predominantly support those patients requiring Pathway 3 (complex care needs, possible CHC eligibility or NWB pathway). A multidisciplinary team (MDT) comprising: nurses, therapists and social care manager will be provided in each location by EKHUFT and KCC and will include, and be supported by, a GP who will lead a weekly MDT meeting and provide medical input and care.
