## Notice

Expressions of interest are invited from appropriately experienced and capable suppliers to commence an engagement with North West London Clinical Commissioning Group (The Commissioners) to provide a patient self- education and empowerment application focusing on Long Term Conditions (LTC), particularly diabetes - to help patients better manage their condition. The Commissioners will be working with the patients to help them access verified and accurate materials about diabetes as a disease, what the patient can do to manage their condition better, dietary advice, and generally improve their health while reducing their burden on the NHS. This is a pilot exercise. As such it will only involve a single Primary Care Network (PCN). This is [Kensal Rise and Willesden West]. The PCN has a population of just under 55,000. The project is focused on patients who are already disadvantaged due to various inequalities. These include some or all of the following factors:- * Gujarati as their first language - English their second. * Mixed levels of digital capability. * Poor literacy levels in some cases. * Lack of ability to effectively manage their own health due to nature of their work (High number of night shift workers in the patient population). * Inability to attend medical appointments due to work commitments. * Poor understanding of the benefits good self-management might bring. * Potential inability to afford data packages leading to digital exclusion. This means the software must have the capability to display educational material in Gujarati. It would be a substantial bonus if it could vocalise the text in fluent Gujarati. In addition, the patients' medical condition will be monitored remotely. This might consist of recording patient's blood pressure, heart rate, HbA1c or cholesterol reading. These readings will be taken using remote devices and the readings will be sent via the internet or other electronic communications means to the practice. It is essential that the software can interface with the practice Electronic Patient Record - which is EMIS in this case. It must have the ability to write the data into the patient and the data be saved. The Commissioners are looking to appoint an innovative, forward thinking company to enter into a relationship. At the moment the project will only run for one year. Therefore, suppliers are asked to complete the attached questionnaire on the basis of the information supplied in this document. Value for money will be a significant factor in awarding the contract.
