Needs analysis, service gap identification, and market engagement have identified that for DCCS to deliver an improved, integrated and improved outcomes-based early intervention/preventative offer to adults the department should put in place a coordinated supplier infrastructure which will move the department towards developing a system approach with consistent and interlinked outcomes which will further promote independence, delay or reduce the development of health and social care needs and reduce hospital admissions. This procurement is splt into 2 Lots: Lot 1 = City Community Connections Services (PS200k per annum) Lot 2 = City Finance Services (PS30k per annum) The estimated contract value given is for the full duration of the contract including the maximum possible extensions.
