## Notice

Delivery of Primary Care Local Services to the CCG GP registered population of North Lincolnshire. Enhancing continuity of care and delivering safe, accessible, convenient and timely local services to the patient closer to home. Service will include: - Providing Care for Patients with Diabetes (including Insulin Initiation, GLP-1 Initiation, MDT and glucose tolerance testing), - Post Hospital Dressings, Suture Removal & Phlebotomy in Primary Care, - Minor Surgery, - Shared Care Monitoring, - Elderly Care Fund, Additional information: Delivery of Primary Care Local Services to the CCG GP registered population of North Lincolnshire. Enhancing continuity of care and delivering safe, accessible, convenient and timely local services to the patient closer to home. The CCG believes that the Local GP federation is best placed to deliver these services on behalf of their member GP practices. All local GP practices who deliver services in the area are members of the local GP federation and all of these GP providers will have access to the services in question. No additional fee for the management of this contract will be available to the lead provider above and beyond the set tariff rates.A single contract will be let from 1 April 2019 for a potential provider to manage the delivery of these services across the three care networks. Any other providers who would like to be included in this process should express an interest through the response form on the eTendering website https://www.nhssourcing.co.uk in order to inform the CCG's commissioning intentions.
