It is evidence based falls awareness and prevention service to improve balance, reduce falls and enable keep people living independently. It aligns with the 8 week CLCH Falls Preventions Service for people with a higher need and risk of falls. The open referral service offers a 38 week programme to build confidence and maintain independence by promoting an active and healthier lifestyle though physical activity for residents in the Tri-borough. The service outcomes are to improve functioning e.g. posture, balance, mobility, orientation and safety awareness and prevent/reduce unnecessary hospital admissions and other acute care as a result of a fall.
