To facilitate the discharge of people to their existing care home placement or to a new care home setting. To help increase the bed flow and status. To reduce the number of assessments an individual has to undergo. To ensure people are discharged to the right setting with complete and up to date information and paperwork. To develop a standard assessment framework for care homes. To improve patient and user experience.
