Enfield CCG through the Better Care Fund Plan has proposed to ensure support and an ongoing point of contact for people with dementia and falls prevention is incorporated into the proposed development of the voluntary sector. The services provided by the voluntary sector will be crucial in supporting patients and their carers. Enfield CCG is seeking a lead organisation, to work in partnership with other voluntary sector organisations, to deliver a post diagnostic support for people with dementia and falls prevention service across Enfield. The provider will support the goal to promote better patient outcomes.
