The service will support clients to maintain health, independence and choice and control over their lives with a particular, but not exhaustive, focus on enabling clients to understand and manage their mental health, sustain their accommodation, maximise their finances, meet their potential aspirations and develop and maintain social contact. This focus will enable clients to live independently, prevent homelessness, social exclusion, relapse and crisis admissions to hospital and enable recovery and rehabilitation.
