PLEASE NOTE, FAILURE TO PROVIDE THE BELOW INFORMATION MAY RENDER YOUR BID NON-COMPLIANT*** Please complete Cost Per Mile and Variable Mileage Rates within the bid form (Appendix B Price schedule) ***Bidders Must provide the following DBS information for all proposed drivers and passenger assistants *** 1.DBS certificate numbers and issue dates AND *** 2. A Scanned copy of enhanced DBS Certificates for all proposed drivers and passenger assistants
