In order to inform this review the Council are interested in understanding what provision there is in the market to offer a brokerage service for people in need of these placements/packages. The brokerage service is envisaged to be a team of people who would form a conduit between the commissioner, care provider and hospital/social care teams, ensuring that placements are made in a timely manner, reducing delayed transfers of care from hospital and ensuring that placements in care homes and with care providers meet service users' needs at an acceptable cost.
