To keep a list of and provide qualified S12 Doctors to undertake mental Health assessment within a week of allocation and return the completed form 4 to the DoLS team within 10 days of allocation. These Doctors will be quality checked and all relevant credentials checked, and evidence collected. A Contract for 1 year with a possibility of extension of another year if assessments are completed successfully within budget and are of high quality.
