This aligns with the National policy drive to develop integrated care systems. This is intended to improve clinical outcomes and patient experience while ensuring that resources are used efficiently and that the system is financially sustainable. The pilot will further improve the model of care. The assessment of NHS Croydon CCG is that the only providers able to establish and coordinate this new service through an integration across the local primary care provider landscape, is Croydon Health Services NHS Trust in partnership with the Croydon GP Collaborative. This contract ensures a continuity of essential service provision to the local population.
