We undertook a consultation period, including a market engagement event, in September to October 2018 which have informed the commissioning plans for carers services. The core aims of redesigning services are as follows: Designing a new model of support for carers based on best practice and which is located in the wider context of the carers pathway as it is envisioned in the future. Identifying carers, especially hidden carers, and empowering them to access services. Meeting the needs of carers through a robust 'carers pathway' in Hackney which provides adult carers with a clear process to requesting and receiving support for their caring needs. Supporting carers to continue their caring role and prevent breakdown. Ensuring equitable and efficient use of resources.
