This programme is for patients who have either had a recent cardiac event or require ongoing cardiac support for an identified heart condition by primary care. Included in this contract will be an Exercise Referral Programme to support patients with long term conditions. This will be delivered in the areas of Orpington, Beckenham and Penge.
