This procurement process is aimed at providing telephone-based behavioural coaching services for adults aged 18 and over with long-term health conditions. The engagement period concludes on 12 January 2018, and the service is intended for delivery in the South East region of the United Kingdom. This early engagement is part of the tender process; however, it is not the formal start of the tender announcement. Your feedback at this point will not have a bearing on any future Tender submissions you may wish to offer at a later date. You will not be disadvantaged if you choose not to respond to this RFI but it will be helpful to understand your views at this early stage, so you are encouraged to respond as fully as you can. The aim of the service is to provide a telephone-based behavioral coaching service to support patients with long term conditions improve their ability to self-manage and help to reduce avoidable accident and emergency attendances and hospital admissions. This will be achieved by offering advanced coaching and motivational interviewing techniques over the telephone. Patients can become more confident at managing those issues that have become a block to attaining and sustaining lifestyle changes that in turn improve health outcomes.
