The CCG is seeking tenders based on a number of EOL services operating in synergy to provide joined up, holistic EOL care for patients. The contract will incorporate the provision of a Co-ordination Hub and Urgent Response Service available 24 hours a day 7 days a week. It will also include provision for specialist palliative care, physical, psychological, spiritual and social support to patients, families and carers. It is anticipated that these services will be provided by a Lead Provider or consortium of providers. In addition to the services being tendered the CCG also commission a range of specialist Hospital and Community based palliative care services are delivered by Sandwell and West Birmingham Hospitals. These services are part of the block contract and will not be included in the scope of this tender.
