## Summary

The procurement process for the project titled "Somewhere To Assess" is being managed by Sheffield City Council, in collaboration with NHS Sheffield Clinical Commissioning Group and Sheffield Teaching Hospitals NHSFT. This initiative, classified under health and social work services, aims to facilitate the timely discharge of hospital patients, particularly those needing assessment in a care home. The tender has been published on 11th June 2019, with a submission deadline set for 11th July 2019. The envisioned contract period will commence on 5th August 2019 and extend until 5th August 2022, with delivery expected in the Yorkshire and the Humber region of the United Kingdom. This open procedure tender encourages wider participation from interested parties.

This tender presents a valuable opportunity for businesses involved in health and social care services, especially those offering care home facilities or related support services. Companies that excel in patient care, rehabilitation, or discharge planning would find themselves well-suited to compete for this contract, contributing to the overarching goal of reducing hospital admissions and facilitating better care for individuals transitioning from hospital to home. Engaging in this procurement process can enhance visibility and business growth within the health sector, given its focus on innovative care delivery models.

## Notice

NHS Sheffield Clinical Commissioning Group (CCG) and Sheffield City Council (SCC) have been working in partnership with Sheffield Teaching Hospitals NHSFT (STH) to: * Help reduce emergency admissions to hospital * Support timely hospital discharge, and; * Wherever possible, discharge people to their own home or normal place of residence. Somewhere Else to Assess is a model that supports individuals who are medically fit to be discharged from hospital, but would benefit from a period of assessment in a care home setting while their long-term care and support needs are identified. This model is a key element of the Health and Social Care vision to: * Develop services that keep people well at home * Reduce unnecessary hospital admissions * Reduce the time people have to stay in hospital when they are well enough to leave and; * Only utilise hospital care when clinically necessary and only for as much time as is needed to resolve the acute need. Assessment beds allow 'future needs conversations' to take place outside of an acute setting, reducing the person's risk of infection and/or decompensation in hospital and ensuring discussions about the future don't take place at a point of crisis when their needs are likely to be higher. The aim is always for people to have an opportunity to return home.
