As part of individuals move on plans the service must include some transition work with appropriate community/floating support services where needed to ensure continuity of support to mitigate anxieties of moving onto independent living and setting up and sustain their new tenancy. All aspects of support within the service should enable individuals to live independently and sustain their tenancy or home and prevent the need for more intensive support. The service must provide person-centred, structured and outcome focussed support based on an individual's identified support needs and work positively and proactively with other appropriate services to ensure those support needs are met in a co-ordinated and holistic way. The goal being to prepare individuals for move on to an independent tenancy elsewhere.
