The overall aim is for the Client to live in a safe and friendly environment where their care and support needs are met, and to enable them to be as active and retain as much independence as possible. The Service Provider will support the well-being and the quality of life of the Client. The Approved List will be used to provide individual client placements through a call off process, as well as discharge to assess' (D2A) beds and Block bed contracts on behalf of the Council and its NHS partners.
