The intervention will be designed to empower people that require support to better manage their long term condition (LTC) through a self-management model. The Programme must be available to people within the geographical area of the Derbyshire ('County'), with the exception of Derby City. In line with the national 'Talking Health' approach the Programme will target people living with LTCs; with face-to-face course sessions being delivered by trained volunteer tutor peers (themselves living with LTCs). The aim is to empower and support people by increasing their confidence, improving their quality of life, helping them to manage their condition more effectively and become an 'expert patient'.
