The requirement is that the individual therapy programme will be formulated with the person, to achieve the following objectives; o return home at the earliest opportunity and or; o increase or maintain independence, choice and control and or; o Instigate planned treatment and care to manage deterioration in health and wellbeing. The Service will therefore strive to deliver a service that is person-centred and delivers clinical effectiveness and optimised outcomes, whilst maximising independence and supporting the reduction of further non-elective acute admissions. The Contract will be for a term of 12 months, with the possibility of extending the term for a period of 12 additional months beyond the initial contracted duration by agreement between the Authority and the provider. The period of the contract will run from 18/11/2019 to 17/11/2020 (with option to extend for a further 12 months).
