This will be for Adults and Children that meet national eligibility criteria for social care and Adults eligible for non-complex health funded Continuing Health Care at home. Support will be for eligible adults and children/young people and will provide personalised care and support to people in their own homes to enable individuals to live independently. The service will also assist carers/parents to maintain their care role.
