The Provider will be responsible for supporting individuals through a rehabilitation model of support, with the aim of them moving on to long term living arrangements. \r The Provider will have demonstrable experience and expertise in delivering person centred support to Service users with a wide range of Mental Health needs and other complex issues. This will include experience of delivery of similar schemes with a focus on Mental Health rehabilitation and supporting Service Users to safely and sustainably move on to longer term situations.\r Providers are expected to support their tender responses with examples and evidence from their own mental health rehabilitation expertise and practice. Specific and tailored responses to the tender questionnaire, should be provided, which clearly show how Providers can meet the requirements of the Council's specification.
